Does anyone have a shell template for a motion to revoke a passport? I need one ASAP.

Just sent you an e-mail with what I've used in the past. The only time I've done it before, the court ordered the passport surrendered and we sent copies of everything to the State Department's Passport Services offices for their action on denial/revocation of it.
