Does anyone have or know where I might find a copy of the official form that is supposed to provided by DPS for registering sex offenders under TCCP 62.051(b)?

You can contact your sex offender rep with DPS and they can provide you with it. If you have the secure access on the sex offender website you can print the form from the website (you will need carbon paper for it though). You can also order them from DPS try calling (512)424-2367 and ask for CR-35.
