I'd suggest that you give Jack McGaughey or Kevin Benton at the Montague County DA's office a call. (940-894-6211).
That office has probably more recent experience in that area than any other I know of.
They exhumed 10 people in 5 days on the killer nurse case that we recently had plead. And I believe they've done a couple of unrelated exhumations since then.
I'm sure they have all the forms and protocols you might need.
