Anyone have or know where I might obtain a form entitled, "Application for the Management of a Person with a Communicable Disease?"

A few years ago, we committed an individual to the state hospital in Rusk (which I think is the closest one to Amarillo that handles communicable disease commitments) based on active tuberculosis. Dave Kemp, the First Assistant in our office, handled the case. His number is (806) 379-2265, and I believe he still has the forms (all the way through judgment).
