Cell phone company realizes it is fake after the fact and cuts off his service, so he goes back to cellular store with a fake social security card (different number than one given before, but fake number nonetheless) to patch things up. Fails miserably. False statement to obtain property or credit or tampering with gov doc?

It could be a third degree forgery or a second degree tampering with governmental record for his conduct when he appears in person at the store.
