Juvenile Adjudication for Sex Crime

I have a person who went to TYC for a sex crime as a juvenile. He is now an adult and registered as a sex offender. He moved without reporting a change of address. How do I prepare the wording for the indictment? I think I saw the wording on here once but I can't find it. Thanks.
