County Hospital has a relief doctor who works on some weekends for the County Hospital. He is paid on an hourly basis. The Hospital wants to have an agreement signed between the doctor and the Hospital that would allow the Hospital to collect reimbursement from Medicare Part B for the physician�s services for the Hospital. Has anyone dealt with this issue before? If you have I would appreciate borrowing a form to use.
