Does anyone have a sample lifetime protective order for a victim of sexual assault (application, temporary orders, and permanent orders)which is authorized by Chapter 7A (and specifically, Art. 7A.07), Code of Criminal Procedure?

I have not had to do one of these before and don't have any sample forms, although, if necessary I can draft one, myself. If, on the other hand, anyone out there has a sample they can send me, I would love to not have to re-invent the wheel... My email address is mrogers@co.orange.tx.us.
