Can anyone provide templates for a DTA and a PC affadavit to back it up? Also, I have seen that a DTA can be requested of the Juvenile Ct. by a prosecutor, but FC 52.015(a) says "...on request of a law enforcement officer or probation officer..." May a prosecutor initiate? Many thanks.
